In the Act stage, providers and patients work together to use the data gathered throughout treatment to assess progress and make decisions regarding treatment.
Have things improved, declined, or stayed the same? If things are improving, perhaps no changes need to be made to the treatment plan. If measure data indicates that a patient is not improving or that things are getting worse, adjustments to treatment may be warranted. Ask your patient to share their concerns, needs, and desires and to provide ideas for moving forward. Then, share your own ideas ensuring that your patient understands how the different treatment options would meet their needs. Discuss the options together to decide on a plan of action that takes into consideration your professional opinion and their desired course of action. Document this plan in the patient’s chart.
