Measurement-based care

Share stage

Components of the Share stage

  • Reporting and discussing the measure data with your patient, ensuring your patient understands the assessment results
  • Working collaboratively to reflect on whether the scores match your patient’s experience of symptoms and progress to gain further insight on how the patient is doing.

Sharing and discussing the measure data

After administering and scoring the measure(s), share and discuss the data with your patient, ensuring your patient understands the assessment results.

Work collaboratively to reflect on whether the scores match your patient’s experience of symptoms and progress to gain further insight on how the patient is doing. If your patient does not feel the scores reflect their experience or if they have misunderstood certain concepts, explore those discrepancies in an open manner and take time to provide education on the measure or measure items.

Document the scores and discussion in your notes and/or patient medical recorder, as applicable, to be used to track progress and for viewing by other providers working with the patient.
