Collect, Share, Act Framework
The Collect, Share, Act Framework is one clinical model for successful measurement-based care (MBC) developed by the Veterans Affairs system and promoted through the works of the Yale Measurement-Based Care Collaborativeopens in new window. Dr. Sandy Resnick and Dr. Jessica Barber’s publication Collect, Share, Act: A Transtheoretical Clinical Model for Doing Measurement-Based Care in Mental Health Treatment (PDF, 632KB)opens in new window can be used as a guideline for psychologists to begin implementing MBC.
The Collect stage involves three components:
- Introducing and explaining MBC to your patient
- Selecting which measures to use
- Administering and scoring the measure(s) on a regular basis
The Share stage involves two components:
- Reporting and discussing the measure data with your patient, ensuring your patient understands the assessment results
- Working collaboratively to reflect on whether the scores match your patient’s experience of symptoms and progress to gain further insight on how the patient is doing.
The Act stage involves three components:
- Reviewing the data trajectory, i.e. how is treatment going? Have things improved, declined, or stayed the same?
- Working with your patient to brainstorm next steps for treatment
- Collaboratively deciding on a plan of action
